Enrollment Information

The dates for fall 2009 registration have been set. Pre-enrollment, for families already enrolled in our program and members of QSBC, will be Monday March 2nd and Wednesday March 4th. This is on a first come basis for everyone and we will start at 9:30 on each day. Open enrollment will be taken Monday March 9th and Wednesday March 11th, and any time thereafter. Class sizes are set and as soon as each class fills up we will begin to put children on a waiting list. ALL paperwork, shot records and registration fees MUST be turned in at the time of registration in order for your child to be placed in a class. There will be NO exceptions and all registration fees are non-refundable. Please note that all accounts must be current to enroll for the fall. All of the necessary forms are available below for you to download at home. This will allow you to have your forms filled out before you come. You can also pick these up at the front desk.

On the day you come to enroll you will need to bring:

  1. A completed enrollment form for each child you are enrolling.                                                   
  2. A completed emergency medical release form for each child you are enrolling.                                                                                                               
  3. An up-to-date copy of your child’s shot record so we can make a copy for our records.
  4. Your $50 registration fee for each child you are enrolling.
