In order to enroll you will need to bring:
- A completed enrollment form for each child you are enrolling.
- A completed emergency medical release form for each child you are enrolling.
- An up-to-date copy of your child’s shot record so we can make a copy for our records.
- Your $50 registration fee for each child you are enrolling.
